Pt has 64635, 64636, 64636.
How would I submit that, as CPT says not -51, and not sure -59 fits.

Was advised to certain insurances, NOT to do units.
 
64636 is an add-on code, so you cannot use a modifier. If you cannot indicate 64636 x 2 on a claim, I would send the claim via snail mail with a copy of the chart explaining what procedures were done. On the claim, I would list it as you did.

You can add 59 modifier to the second Add - on code If insurance not accept to bill as Units.
Otherwise insurance will deny 2nd unit of 64636 as duplicate (Usually 51 modifier cannot be append for Add - on Codes).
