We have been denied payment for some AICD's coded with 33249 saying that we need
to add modifier Q0, which is for clinical studies. Our patients aren't registered in a
clinical study or have any of the approved Dx. I'm a new coder and don't know what
to code to get these paid for. Any suggestions?

only Medicare accepts the Q0 modifier your patients do not have to be registered with any clinical studies those codes for Defibrillators automatically get that modifier when it's a Medicare patient all other carriers deny that procedure code with the Q0 modifier.
