Can 36558, 36010 mod 51, 77001 and 76937 be bill together or it will be denied as bundle... :confused:
 
Normally you would bill 36558 & 77001-26, but without report hard to know. The only time you would be able to bill 36010 is when its medically necessary or tried to place ash cath on one side but unable to so went to the other side.

The 36558 inlcudes the catheter placement..

Really need the op note but this should help get you in the right direction. :)
