If the provider say the patients Hep C is undetectable, would I code for the Hep C or history of?

I think you'd need to ask the provider this question. It's really a clinical judgment to say what 'undetectable' means as far as status of the patient's disease. The provider would need to make that determination as it's outside the scope of coding. If the diagnosis isn't clear, query the provider.
