Provider documented that Adult Hep B vaccine was unavailable for patient , equivalent dosing with two Peds Hep B vaccines were given to patient.
Should CPT 90744 be billed twice or is it okay to use the adult CPT code 90746 due to patient age?

You would want to use the 90746 as the dual dose above equals to the adult dosage. Make sure that the correct NDC# is attached to this code to indicate the appropriate vial that was given to the patient. Depending on the carrier you may get an initial denial as the NDC may be questioned, but you will be able to appeal with the medical records to address and explain the discrepancy.
