Our office billed a Hepatitis C Panel for HIV testing (LAB). We used to bill dx V73.89. The lab indicated that this code is no longer accepted/ found to be medically necessary per Medicare guidelines. Anyone have this issue?

Also, is HPT C Panel included in a physical exam or should it be billed separately?
