Resident did all documentation and teaching physician added, "I discussed the patient with the resident and agree with the documentation and plan of care per resident note."

So.......

Being that the TP didnt state he/she had 'seen and examined' the patient, the WCC is not billable. But is the sick visit billable under the primary care exception??
