I've only been coding GS a short time (just under 12 mo) and I really would just like a few pointers from any guru's out there in relation to hernia's.

Basically, I want to make sure I capture the correct code, and the physician does not always document if the hernia is incarcerated or strangulated; I see things within the documentation like, "the hernia contained omentum," is that suggestive of incarcerated or strangulated hernia?

Any key words or phrases I can look for to identify and differentiate between reducible or strangulated would be greatly appreciated.
