From the dictation:
"I found an additional fascial defect at the umbilicus inferior to the main hernia defect [ventral hernia]. The two defects were joined creating one abdominal wall defect for repair."

Would I code the ventral and umbilical hernia repairs separately, or can I only code the ventral hernia repair and mesh placement because they were combined? If I'm only coding the ventral hernia repair, should I append modifier 22?

One repair - one code

Per the physician's own documentation there was only one repair. I would not even add a -22.
