Wiki hernia repair with 3 separation incisions

I have a patient who had incisional hernia. She has had repeated laparatomies and there are three incisional hernias now. The physician repaired all three of the hernias with mesh and there were three different incisions (one for each of the hernias). I keep getting edits that will not accept all three entries even if modifiers are used. I am coding on the facility side. Does anyone have any suggestions on how to proceed in order to capture all three repairs?

Small bowel resection

Not sure how to bill this op report? Patient had small bowel resection x 3 and anastomosis x 2. Does any one have any input? I was thinking 44120, 44121 x1 44121-52 x 1? any help would be great.
