I am going to ask silly question.

Is it possible to code 272.1 and 272.2 together?
Doctor documented like it is 2 different issues.
272.1 (Change RX tricor to lopid)
272.2 (change lipitor to crestor)

I think we can code like if we go thro' ICD tabular list and our documentation matches the criteria in ICD 9 CM' Index & Tabular list then we can!!!

Is it possbile to share "documented like it is 2 different issues" ???
