Per FSCO guidance, billing of medications over $99,999,99.00 the claim must be split over multiple claims that include modifier -76. Of course the JZ or JW modifier would be attached dependent on the situation. Is anyone aware if Humana Medicare follows the same guidance for claim submission on medications over $99,999,99.00. Please advise.
