For ASC

If a Medicare 68yr patient is scheduled for a screening colonoscopy, family hx of colon cancer. and they find hemorrhoids, small diverticula throughout colon and luminal tortuosity of sigmoid colon, no polyps, no biopsies.

Professional/facility
Do you code G0105
Dx Z12.11, Z80.0, K64.4, K57.30

or

Professional/facility - due to findings
45378 - PT
Dx Z12.11, Z80.0 K64.4, K57.30

Per my understanding, the findings should not change the reporting of the procedure code. The only difference between the descriptors of G0105 and 45378 is that 45378 can including the collection of samples by brushing or washing, when performed, whereas G0105 does not. So if no samples were collected and no other interventions performed, then the procedure is still just a screening colonoscopy reportable with G0105.
