What is your overall process for auditing high risk diagnosis for risk adjustment? I have identified three areas that need further review, but how do all of you determine how many charts to review, process for reporting findings, etc.? For acute CVA, I have over 400 codes in a physician's office alone. I really need this info ASAP. Thanks.
