Does a high risk diagnosis alone support applying modifier 22 to the global ob charge (59400)?
 
No. Appending a modifier -22 on a surgical procedure indicates that the surgery was significantly more difficult due to a situation not normally present when that procedure is performed. The operative note must state what challenges were present, what was done differently to compensate and why the surgery was significantly increased. Simply reporting a diagnosis would not illustrate the complexity required to append the -22.
