Wiki highmark denial

I have recently received denials from Highmark BC for assists by a mid level (PA or NP) stating that the records submitted (operative report) "did not indicate how the assistant at surgery actively assisted the operating physician performing the procedure". I tried to call Highmark and get some clarification on what exactly they want but I was told they could not tell me how to bill. Has anyone else had a similar issue?

The surgeon needs to prove the necessity of the assistant by documenting what duties the assistant performed.
