When submitting an EOB for an appeal back to the insurance does anyone
black out the names and identifying information of other patients
or do you send the complete EOB with all information for all patients showing?

Is this called a release of protected health information since the insurance has it, as they provided us with that information in the first place?

I have ALWAYS blacked out the other patients information on the EOB for two reasons. The first reason is they do not need to see all of the other information and, two, they can see immediately where the information is. I do not believe it is a form of HIPPA violation as they were the ones that sent it to you. Hope this helps.
 
I always block out the other patients information as other pt information is not needed to be recognized
