Wiki History of an E&M; Time based coding

In the history component of an E&M, can different constitutional elements be counted towards a level? E.G., if a patient has fever, weight loss, stuffy nose, can we count all 3 towards the total elements for code determination?

Also, on the face to face time based coding include viewing documents if still in with the patient, or are there other factors?

I'm confused by your question. But ...
You ask E.G., if a patient has fever, weight loss, stuffy nose, can we count all 3 towards the total elements for code determination? I'm going to assume you are looking at the HPI. You would have LOCATION for "nose," and ASSOCIATED SIGNS/SYMPTOMS for "fever, weight loss."
