Just wondering when a person is considered to have history of ca vs active ca. Do you consider 3-5 or 7 years. I am just trying to take a survey ands if there is any documentation you could guide me to.

coding clinic M-J 1985 is a good resource. A cancer is hx of once all treatment has ceased and there is documented no eveidence of disease. Breast ca and prostate ca are a little peculiar in that they are treated long term with drugs like tamoxifen and lupron, in some cases up to 10 years. If the patient is one of these drugs or the equivalent then the ca is still active.
 
yes, I was also of the understanding that you would use the history code when treatment has ceased - I believe that there may have been an article addressing this in a recent EDGE
