History of uncertain behavior

Does any know what code you use when there is history of a rectal polyp that codes out originally to uncertain behavior. The pathology was Villous adenoma. The lady had surgery and now is back in for a follow up sigmoidoscopy. Can any one help on what code to use? We were tossing around V12.72 or V10.06 but neither tell the complete story. Talk to the physican to do benign or malignant and he said it was both?

Any help would be great!

villous adenoma is a polyp and therefore benign. I would use v12.72.

The other code that I sometimes use is v45.3 - intestinal anastomosis state.
