At what point would a patient's diagnosis change from current to personal history of for DVT's, CVS's, PE's. These patients are on chronic coumadin therapy and it is being questioned when to start using the V codes for history of instead of the codes for current conditions. My first thought is that the doctor should be given the option of either dx. and let him decide.
