Wiki History & Physicals chargeable if admitted from ER

If a patient comes into the ER and then becomes admitted as an inpatient and they do a procedure the same DOS, do you charge out a (99212-99215 or 99201-99205) for the History& Physical since the procedure isnt planned?

Really confused on these H&Ps.
 
if your provider is the admitting provider then you need an initial inpatient level 99221-99223 with the 25 modifier. If your provider only sees the patient in the ER and then performes a procedure then you use 99281-99285 ER levels with the 25 modifer. Now it is a 25 modifier if the procedure performed has 10 days or less in the post op global if they have more than 10 days then you would use a 57 modfier.
