Wiki HLP with Modifiers on IV INfustions

I need some clarification:

For example:
96372
96374
96375
96361
Which ones would need the modifier and would it be a modifier 59 or 76. In this case the patient was seen only once that day.
Here is another example at 10 am:
93674
96375
Then patient comes back at 2 pm:
96375x2
Then patient comes back at 5 pm:
96375x2
I am assuming the 2 pm and 5pm would need a modifier 76 since it is a repeat same day.

For the first scenario,

96372 need to bill with modifier 59 based on NCCI edit

for the second scenrio, there is no need of modifier.
