Two questions about Holter monitoring coding:
1. Can a physician bill for an office visit at the same time as billing for 93225 and 93227? Are any modifiers required for payment from either commercial carriers or Medicare?
2. Will Medicare reimburse 93226 to a Holter scanning company while at the same time reimburse a physician for 93225 and 93227? Or can only the physician bill 93224 as the global charge to Medicare?
