Question on billing Care Plan Oversights. I just started a new position and they are backed up on the CPOs, and as I am vaguely familiar with them, they have given them to me to sort out. However, I am noticing that the trend seems to be the doctors sign and bill the 485 form for certification and re-cert, but then on the CPO monthly log sheet, the 485 form is the only documentation they have to bill their 30 minutes. Am I correct in thinking this isn't sufficient? Any input appreciated!

It just so happens that this month's AAPC Coding Edge Pg 26, has all the information on this.

The 485 form itself really doesn't prove time.

The article had a great "form" that can be used to document the time spent on the CPO.
