When a patient is delivered at home what are the appropriate codes? Code sets?

I am surmising that dependent on the care of the patient antepartum this could include the codes 59400, 59409, 59410, 59412, 59414, 59425, 59426, 59430 or 59610, 59612.
Would codes 99500, 99501, 99502 be appropriate in addition to the global package?

I would have to say that 99461, 99464, 99461, 99477 would all be excluded unless the mother and or the baby ended up admitted?

Does anyone have experience with this. I am from a Blue Cross Plan. Thanks Sara :o
