What is the correct date of service to use for billing a completed Plan of Care for a Medicare patient receiving home health services? Example: the physician completed a cert and a recert, but signed both forms on the same date. Obviously, the verbal orders were provided to the HHA before the services were provided, but if I use the signature date as the DOS, one will deny as a duplicate. I've checked the IOMs, but cannot find regulations addressing this scenario.
