If a patient has been on anticoagulation therapy for an extened period of time since being diagnosed with a pulmonary embolism, what is the correct ICD9 code to use for the home INR charges? Would you bill with 415.19 or
V12.55?
 
Does the patient still have a PE or is this documented as a history?

I would go with V12.55 if documented as hx of, otherwise, I would go with 415.19 or query the provider to ensure proper ICD-9.
