Could someone hlep us for the most appropriate code??
PT made a emergency call to DR after the office hours.
Dr. made home visit and called Ambulance to ER.
Dr. attended at ER and admitted patient. Can we code with home visit, admission and 99058 or 99060?
Thank you

CPT guidelines

CPT guidelines tell us that if the patient is admitted on the same date of service as other E/M services provided then only the Initial Hospital Visit is coded with the work of the E/M service being taken into account to determine the level of 99221-99223.

We also know that you can use the Prolonged service code for additional face-to-face time spent with a patient over and above documenting the E/M service. Your scenario sounds like this may be a possibliity.

OR ... was the patient critically ill? Did the physician provide critical care services? Then use codes 99291-92 depending on total time spent throughout the day providing critical care. (I would not code the initial hospital visit in this case, but just code 99291-92 depending on total time spent.)

Hope that helps you.
