hello all,
how do we code home visit for 2 hour long. example, if some provider went patient's home to see and spend 2 hours discussing all chronic condition how do we code this? do we code 99350 for 60 mins the highest level or do we code by the MDM. please help.

Home visits do not fall under the new E/M coding guidelines that apply to office visits. You would need to code this using the elements of history, exam and MDM, unless the provider has documented that more than 50% of the total time was spend in counseling and coordination of care in which case you may use time for coding the level.
