A patient entered the hospital at 35 weeks. She remained hospitalized for seven days
before delivery via cesarean section. Is "57" the proper modifier to append to the E&M
hospital days prior to delivery?

Only to the day the actual decision to do the c-section was made. The other days prior are just billed as inpatient days no modifier. Just be sure the diagnosis coding reflects the complication she was hospitalized for.
