Wiki Hospice- code for Dr's time for orders

When a patient is on hospice, how can we code for the time the Dr spends writing orders, adjusting medication, etc.? Sometimes the patient in hospice needs to have meds changed and the Dr. is spending time relaying orders to nurse but how can we code for that? Thanks
