Wiki Hospice Modifier within Hospital

Please help me in figuring out what modifier to use if a patient was inpatient in the hospital and was discharged to "hospice" but this hospice is not outside the hospital. It is within the hospital in a designated area made for hospice patients only. So what modifier would I use if the patient has private insurance? We are using the Inpatient Discharge code but the patient is being discharge from Inpatient status at hospital and just moved to Inpatient hospice. Can't use the same initial admit....confusing to the payor?!
