I work for a family practice and I am having trouble with specifics on billing hospice when the patient is seen in the office and nursing home. I would love to speak with someone who has experience and can clarify a few things for me! Thanks!

I know the difference in the modifiers, my question is do other offices have a list of hospice patients and their admitting dx? If this dx is documented in any of the note does that mean we should bill the hospice at that point? What if the physician is the attending for the hospice?
