How do you determine the E&M Level when a patient is an inpatient, and the hospitalist stops in to check on the patient? They are not managing the terminal illness. They are making sure the patient is comfortable. Do you still consider the terminal illness high complexity and if the patient is on comfort meds--depending on what they are--high risk and the hospitalist would bill a 99223?

Or

The patient goes into the ED, the ED physician makes the decision to put the patient on Hospice and the patient is admitted. How do you code this type of scenario? Is the complexity still high? What about medication management?
