If a patient has already been discharged and the physician only has to fill out paperwork and does no face to face with the patient, can 99238 or 99239 be charged? I don't have alot of experience with these codes and would appreciate anyone's help.
 
If the physician is just doing the paperwork, besides the nurse, who did the instructions to the patient and/or family? I would think that the physician would need to assess the patient/family's readiness for discharge by doing a face to face. Discharge involves coordination of care so that would entail some face to face in my opinion.
