How do I charge for a discharge when it is done over the phone.
 
You don't. This would be just a telephone order.

Was there any other work done that same day when the doc was actually at the hospital? if so, and he just gave the final "ok"...maybe he was just waiting on one last lab or something...then maybe you can bill a d/c code.
