can any one tell me if it is true that when a patient walks into a hospital outstanding facility etc...that the script has to have written dx example chest pain and not just a dx code on it to be accepted for services ?

Yes you must have the referring providers diagnosis in their own words not a dx code. The possibility of that code number to be incorrect is too great. We always requested a copy of the office note to be faxed over before the test could be performed.
