I work in a hospital outpatient department and we are provider based. We charge a facility E&M code for each patient visit that is not infusion, injections or treatment related. We are reviewing the system we have in place to assign the E&M level. We currently use a point system for the nurses to capture what they are doing and that contributes to assigning the code. This document has not been updated in 8 years. I have been in the department for 4 years and a dedicated coder for the last year. Does anyone know if there are current publications that I can review on how to assign points to our services? We have a good foundation; I just need to ensure that I'm not missing something that is a requirement for PBB or identify something that we are missing that could be included in our system.
