Hospital stay less than 24 hours

It has always been my understanding that if a patient is admitted and discharged on two different dates even if it is less than 24 hours, then the provider should do an H&P and D/C summary; however one of my providers thinks that if the admission is less than 24 hours he can include the admit and d/c on the same note. Shouldn't the provider be doing separate notes since it is different dates?

This sounds like a documentation issue that the physician needs to find out from the hospital. Hospitals have their own policies on H&Ps and D/C summaries.

You can't bill the 99234-36 range because they are on separate DOS. This is clearly stated in the section guidelines in CPT.
