My physician was called in for a consult on 4/1 for abd pain, saw the patient for subsequent visit on 4/3, performed the procedure found gastric hemorrhage. Doc wants to bill 99232 for 789.00 and the procedure with the hemorrhage code. My question is, do we bill for the symptom after we know the result? Shouldn't the visit be included in the procedure?
 
this depends on when the surgery was done. same day? decision made for surgery during the visit?

if same day - E&M - Mod 57 (decision for surgery), and symptom dx, then after surgery the surgery CPT code with surgerical findings DX.
