I work for General Surgeons and this type of scenario happens quite a bit and I don't know how to code this and not get a denial every time. We have a patient in the hospital and the doc coded 99223 the first day and documents that the patient has septic shock with nonspecific etiology. The patient is critical and needs to be rehydrated. He recommends rehydrating the patient and he will reevaluate her in the morning to determine whether surgery is still a viable option.

Next day, doc goes in and sees the patient and makes the decision for surgery and performs the surgery same day. He codes 99232-57 plus the surgery codes.

How do I bill for the 99223 and not get it denied as global?
