I have a resident who's primary dx is acute cholecystitis s/p cholecystectomy... secondary dx is cholelithiasis, choledocholithiasis...

do i code all of these separately... or do i just use 574.60 and V45.79?
 
If the patient is status post cholecystectomy, then there is no way to have acute cholecystitis, or cholelithiasis, or any condition pertaining to the gall bladder. if this is post operative then al you use is a post op floow up code or an aftercare code, both are V codes , you do not use the pre operative condition once the condition no longer exisits. And once you have a cholecystectomy, those conditions do not exist.
