Scenario: urgent care patient comes in with 30 diagnosis other than Dx for reason for visit such as sinusitis. How do you know what codes to code or do you
code them all?
 
With EHRs providers like to copy and paste the ENTIRE problem list and I've seen some diagnosis' listed that are several years old that haven't been addressed in as many years! It is only appropriate to add a dx if it has been addressed (ie. currently being treated with medication, labs and/or X-rays being ordered or reviewed) or if it it has an affect on the treatment of the current issue.
 
Thanks carps14. Being new at diagnosis coding I'm trying to understand how to HCC code from the long list. How to narrow down a long list of Dx that matter. Any advice?
