How many charts do you code per day for ER/ urgent care?
I usually code 100 X day
 
Our productivity standards for outpatient facility ED/Urgent Care coders is 14 charts per hour. This includes abstracting primary and final diagnosis, indicating the correct discharge disposition, listing all current chronic conditions and pertinent history or status codes, determining correct rendering, and reviewing documentation for the presence of orders and physician procedures (and coding the correct CPT). This is done through an encoder (3M), and reviewing electronic records.
