Wiki How many notes do I need to look at???

I am being told that for the professional charges in the EC we need to look at all the consultations done and pull all of the dx codes from them and add them to the EC charges. I might understand this for the facility side, but how can I do that for the professional side if they aren't in the EC docs note?
