Wiki How often can you bill Subsequent swing bed days for the Physician?

I work for a CAH and we have recently started billing swing beds but I am a little confused as to how often I can bill the Subsequent days. :o We have some Physicians that will visit their patients daily and some every couple of days. Do I bill every visit or is there a limit per week? If anyone could please help me figure this out I would greatly appreciate it. :D
