How often must the MD see the patient to continue Home Health/Hospice Orders?

How often must a physician see and examine the patient before certifying and writing home health/hospice orders? CMS states that the plan of care must be reviewed and signed by the provider at least every 60 days, but Icannot find anything that states how often he must actually SEE the patient.
