You can use only one encounter code. You can use all 5 encounters to determine the level without counting any point more than once.

That is an add code and must be appended to the subsequent level code. To use that the provider must document total face to face time plus any relevant floor time on each encounter, then you take the level that you have met using all the visits. Take the time for that visit level and subtract it from total time spent, if the result is 30 or more then you may append the 99356.
