Also the medical record says nephropathy but it is not linked as an underlying manifestation of DM. Please any suggestions.

If the documentation does not specify the neuropathy as a manifestation of the DM then you cannot code it as such. You will need to have the physician amend the documentation or code it as it stands, which is neuropathy and DM uncomplicated.
